I also use WinPatrol (the free trial, for now,) TuneUp Utilities 2008, and HijackThis. Am I being really paranoid, or should I also download another free anti-virus or anti-spyware/malware utility?

Windows Defender is only a antispyware scanner so yes you'll need a antivirus software.
To be honest, I don't recommend AVG but rather one of these two:
Avast! Home Edition

Avast! scored Advance on http://www.av-comparatives.org while AVG Anti-Malware (Antivirus and Antispyware combined)scored Advance+ (not Advance) but please remember that it tested the Anti-Malware not the Antivirus so it's uncertain what the antivirus alone recieved.
AVG is simple and easy to use but I recommend avast! out of the two for it's multilayer shields and protection. Avast! does require a license key that expires after 14 months (if you're using the free version of avast! you can re-register for another key for free).
If you're an advance user, you should also look into AntiVir Personal Edition Classic. It's free and it has a very high detection rate but a bit more false positives from the other two above but never the less it's detection is more powerful.
